Purpose: Many customers require our W9 to set us up as a vendor and submit payment via WIRE/CHECK/ACH. Please note that customers can also download a copy through a link available on all WIRE/CHECK/ACH invoices that are issued.

Vendor Form (SIGNED)

Purpose: To ensure quick processing of customer vendor forms. Please inform customers that we cannot fill out customer specific forms, but this document should include all needed details. These forms are generally required by organizations paying via WIRE/CHECK/ACH.
